Why did USA fail in its initial response to the coronavirus outbreak?

Among the salient points raised in this half-hour BBC interview (linked below) are:

(1) Bureaucratic delays in testing

(2) A failure to learn from an earlier warning

(3) Downplaying the seriousness of the outbreak, and

(4) Inherent structural weaknesses within their largely privatised healthcare system:
- - high costs of consultations and tests
- - 27 million people without medical insurance
- - 25% of workers have no paid sick leave.
